I had an actual serious topic post coming together, but it's going to take more time than I want to take to get out. So I'll just put out the main point.

I think people conflate blame with cause too much. I wish they wouldn't.

(If a plane crashes because an uncontained engine failure severs one or more hydraulic lines and leads to a loss of control, it's more important to me to be able to figure that out and look at how to prevent it from happening again, and not important at all to me to blame the engineers who designed the thing. Thankfully, the NTSB feels the same way; I only wish I could say the same for the French.)
